Bride’s Guide: How to Change Your Last Name in the Philippines


These days, changing your last name can become a highly-debated topic. Many people view changing their last name as the first step to building your own family and making a home with your partner. On the other hand, many also view it as an outdated custom; one that literally demands giving up a portion of your identity. Most, though, simply view it as tradition. Regardless of which camp you fall into, the decision of whether or not to change your last name is one that every bride encounters. It can prove to be quite a complicated decision to make, but fear not-- we’re here to help you every step of the way. 
Here’s every bride’s guide to changing your last name. 
Step 1: Acquire a Marriage Certificate.

Step 2: Go to the necessary government agencies to update your information. 
This is where it gets a bit more complicated. But fear not! We have put together a comprehensive list of requirements and the steps you need to take in order to update your PhilHealth ID, Pag-Ibig, Passport, Driver’s License and Bank Records. 
PhilHealth
Requirements: 
  1. Marriage certificate (photocopy & original)
  2. Old PhilHealth ID
  3. Valid ID
Step 1: Fill out the PhilHealth Member Registration Form (PMRF) available on the PhilHealth website. Be sure to check the “Updating” box found on the upper right corner of the form. 
Step 2: Submit the form to the nearest PhilHealth office to you. 
Pag-Ibig 
Requirements:
  1. Original copy of marriage certificate.
  2. Valid ID (Drivers’ License, Passport, Pag-Ibig Loyalty Card, Company ID, SSS ID).
Step 1: Fill out the Members Change of Information Form (MCIF) available on the Pag-Ibig website. Be sure to complete only the portion/s that need to be updated. For the rest, you can just put N/A. Also, don’t forget to write in BLOCK LETTERS LIKE THIS. 
Step 2: Head on over to the nearest Pag-Ibig office to submit the accomplished form. 
Passport
Requirements: 
  1. Original marriage certificate
  2. Current passport
  3. Photocopy of your passport’s biodata page
  4. Two valid IDs with a photocopy of each. 
  5. Proof of payment
Step 1: Book an appointment at the Department of Foreign Affairs website
Step 2: Fill out a copy of the application form found on the DFA website. 
Step 3: Pay the processing fee at an authorized payment center near you. For regular transactions, you will be charged a fee of PHP 950.00. If you want to opt for quicker processing time, you will be charged PHP 1,200.00. It is also important to note that some payment centers may charge a convenience fee of PHP 50.00 per transaction. 
Step 4: Present all requirements, together with the accomplished application form, at the DFA office where you have confirmed an appointment. 
Step 5: After processing, you will be given a receipt. Be sure to keep this receipt, as you will need it to claim your passport upon its release. 
Driver’s License
Requirements: 
  1.  Marriage certificate (original and photocopy) 
  2. Current driver’s license
Step 1: Fill out a copy of the application form found on the LTO website. 
Step 2: Head over to the LTO office nearest you, and submit the form along with the necessary requirements. 
Bank Records
Requirements: 
  1. Marriage certificate (original and photocopy)
  2. Valid ID with your married name. 
Step one: Visit the bank of your choice and simply inform the teller that you would like to have your information updated.
